From what the back of the card says:

Step 1 - Create online account on your PS device or use existing account
Step 2 - Select PS store icon on the PS4 home screen or PS3 XMB
Step 3 - On PS Store, select "redeem code" at the bottom of the menu on PS4 or PS3 system
Step 4 - Enter the code.
Step 5 - Once the code has been entered correctly select Continue on the dialog box from the PS4 or PS3 system
Step 6 - Select "confirm" to accept the trial code terms of service
Step 7 - Select continue to complete code redemption on PS4...select "proceed to checkout" to complete code redemption on PS3
