I am still so lost to understand how this got a PSA 3. Could it be that a noob grader didn't grade he perforation correctly. Surface is clean. What do you guys think? Am I just way off?

Card is miscut at top (has part of the card above it showing). Normally PSA would give it a (MC) qualifier, but they are trying to reduce their use of qualifiers. So it could have been a 5(MC) and they awarded it a 3. If you think of it that way, then it's likely there is a surface wrinkle they spotted that brought it down to a 5. That's pretty easy to do on a card where you need to fold it multiple times to correctly separate along the perforations.
Did you try holding it at an angle under a bright light source to see any surface wrinkles on front or back?

S.I. cards are printed on super thin stock. Really easy to pick up a wrinkle.

I was about to agree with the OP but I see the miscut. I have PSA 10 perf cards but the miscut is pretty clear.

You have a PSA 10 of this card? I have not seen one that was not offset, I did a ton of research on this card and almost everyone I see has the bleed over at the top.

The post you quoted does not say that. It says "I have PSA 10 perf cards..."

Not this card, but a perf card (2004 Oregon). Just saying that to mean perf cards can get 10s.
